We have a fantastic opportunity for a Patient Transport Liaison Officer to join our team at Royal Stoke University Hospital on the UHNM HDU Contract. The Patient Transport Liaison Officer will support Operational and Call Centre Teams by providing onsite assistance, resolving patient transport issues, and acting as a second escalation point within the hospital for transport-related concerns. This includes coordination with Track and Triage, Occupational Therapists, and Care Teams to ensure smooth discharges and communication with patients about their transport bookings. The role involves working Monday to Friday in shifts (0800-1600, 0900-1700, 1000-1800), totaling 37.5 hours per week.

Main Responsibilities:

  • Maximize patient movement using available resources
  • Coordinate with Trusts, control staff, and site teams for effective communication
  • Prioritize patient care and provide professional solutions to challenges
  • Handle inquiries and escalate transport issues
  • Participate in bed meetings to forecast transport demands
  • Act as escalation point after CCO for all service users

Additional Duties:

  • Work with controllers to ensure timely patient transfers
  • Communicate with hospital staff regarding patient readiness and travel arrangements
  • Monitor Cleric Booking System for updates and accuracy
  • Assist with property assessments and system updates
  • Liaise with patients experiencing delays or issues on travel day
  • Provide feedback to improve services and act as escalation point for delays or issues
  • Support hospital crews and control team in prioritizing and managing patient transport
  • Complete journey documentation and audits
  • Relay critical information during serious incidents
