What is The Deal for you ?

  • Flexibility! You can work your full time hours over 4 days and enjoy a long weekend, or work over 5 days to accommodate your other commitments.
  • Hybrid Working! Do you like to work from home? Or do you prefer being in an office? With this role you have the option of working from home or from our regional offices – or a blend between the two!
  • Salary! £36,000 – £38,000 + £3,000 car allowance

As a Complex Care Assessment Practitioner, you will lead the assessment and development of personalized care plans for individuals with learning disabilities and mental health issues. Your role will involve:

  • Person-Centred Assessments: Conducting thorough assessments to identify individual needs and outcomes.
  • Collaboration: Working closely with clinical teams and external professionals to ensure comprehensive care.
  • Quality Assurance: Leading quality evaluations and ensuring compliance with regulatory standards.
  • Advocacy: Being a strong advocate for the individuals we support, ensuring their voices are heard.

Essential Criteria

To be successful in this role, you will need:

  • Education: Minimum of level 5 qualification in PBS, and willing to work towards a level 6/7 as part of the role
  • Experience: Proven experience in care and support assessments, particularly with individuals with learning disabilities and/or mental health issues.
  • Knowledge: Understanding of Positive Behavior Support, Transforming Care Partnerships, and relevant regulatory environments.
  • Skills: High-level communication skills (written and oral), strong IT skills, and the ability to manage complex assessments.
  • Competencies: Ability to maintain focus during assessments, manage emotionally stressful situations, and build strong relationships with stakeholders.
  • Qualifications: Trusted Assessor qualification (England only) and experience working with Local Authorities and public sector organizations.
