Guy’s and St Thomas’ is among the UK’s busiest and most successful NHS foundation trusts. We provide a full range of hospital and community services for people in south London and as well as specialist care for patients from further afield including cancer, renal, orthopaedic, respiratory and cardiovascular services. Our community services include health visiting, school nursing and support for families of children with long-term conditions. Our adult community services teams deliver care at the heart of the local communities we serve, working in partnership with GPs, local authorities and other healthcare and voluntary sector organisations.

Job overview

We are currently looking for nurses to join our Neighbourhood Nursing teams across Lambeth and Southwark areas. We are one of only two community teams in the UK to receive an outstanding status by CQC. We ensure that our staff are supported and are the only Trust in the UK to offer a Transition Programme which gives our nurses the knowledge and skills to take their care to the community.

Main duties of the job

  • The post-holder will be a member of the Neighbourhood Nursing (District Nursing) Team and will participate in the clinical case management of patients who have a wide variety of complex, high-risk and supported/self-care conditions.
  • Working directly with the Team Leader, the post-holder will help ensure the provision of a high standard of nursing care and health promotion.
  • The care will be provided in a range of settings including patients’ homes, residential homes, nurse led clinics and GP practices.

Responsibilities include:

  • Holistic assessment, planning, implementation and evaluation of a plan of care for patients with long term conditions, tissue viability, continence, palliative care, and promoting/facilitating patient independence.
  • Assists in the delivery of National Service Frameworks.
  • Providing public health and health promotion programmes to improve health and reduce inequalities.
  • Maintaining contemporaneous written and computerised records.

Person specification

  • Experience in care of people with long term conditions.
  • Experience of teaching and assessing students and supervision of staff community nursing experience.
  • Ability to communicate effectively at all levels.
  • Able to work with minimal supervision.
  • NMC Registered nurse, with active PIN.
