We are looking for an Intermediate Care Senior Social Worker for the Central Manchester Community Hospital Discharge To Assess team. The focus of the role is to achieve outstanding outcomes for all our citizens requiring social work assessment and care planning to ensure safe discharge from Central Manchester Intermediate Care Unit.
We are an integrated care team and works 7 days service. This is a busy and focused team, and the role holder should be a critical thinker, with the ability to manage risk and need, whilst maintaining flow through the system.
HDTA Central team is a stable and fully staffed team, and we are looking for a confident and committed senior social worker to join our management team.
The ideal Candidate should have extensive experience and skill in adult social care, and have good working knowledge of Social Work procedures within Hospital Discharge to Assess. Experience of working in intermediate care desirable.
The ideal candidate will have both formal and informal experience of supervising social workers and care assessors. Candidates should have the BIA or Practice Educator qualification or be willing to work towards these. They should be a competent user of LiquidLogic system, have generally excellent IT skills across a range of systems.
