Job Title: Nurse Case Manager (Tracheostomy Experienced)\\nLocation: North London & South London (Hybrid – office, home and community-based)\\nSalary: Up to £45,000 per annum (DOE) + Oyster Card + mileage reimbursement\\nContract Type: Permanent\\nHours: Monday to Friday, 9am–5pm (Full flexibility considered); one evening per fortnight as part of the on-call rota\\n\\nOverview:\\nWe are seeking 5x experienced and compassionate Nurse Case Managers to join a growing healthcare team, supporting clients across North and South London. This is a hybrid role combining office work, home-based duties, and community field visits.\\n\\nAs a Nurse Case Manager, you will be the primary clinical lead for clients with complex care needs, working face-to-face with clients and their families to develop personalised care plans.
