Pay rate: £24.59 per hour

Liverpool City Council is seeking a dedicated and experienced Social Care Assessor to join our Adult Social Care & Health team on a part-time basis. This is a fantastic opportunity to contribute to a strengths-based, person-centred approach to social care with a focus on reablement.

In this role you will:

  • Carry out social care assessments and develop support plans that promote independence
  • Work directly with individuals, carers, families, and partners to coordinate care
  • Manage your own caseload and ensure timely reviews and updates to care plans
  • Apply a reablement-focused approach to all interventions
  • Use Liquid Logic to maintain accurate case records and reports
  • Work collaboratively across multi-disciplinary teams to improve outcomes

Successful candidates will have:

  • Experience working in a social care setting with a focus on reablement
  • Relevant social care qualification
  • Work in a Social Services Directorate or other Statutory or Voluntary Agency
  • Work with a range of client groups within an Agency
  • Confident and competent using Liquid Logic case management system
  • Ability to deliver strength-based assessments and outcome-focused care plans
  • Good understanding of adult social care legislation and safeguarding practice
  • Strong communication, organisation, and partnership-working skills
