Part-Time | Agency RoleAre you passionate about promoting independence and supporting adults to live their best lives?  Liverpool City Council is looking for a dedicated and experienced Social Care Assessor to join their Adult Social Care & Health Team. This is a fantastic opportunity to work within a strengths-based, reablement-focused approach that places the person at the centre of care.?? Your Role: As a Social Care Assessor, you’ll be part of a professional and supportive team working to make a real difference in people’s lives. You will:

• Carry out strengths-based assessments with adults and carers
• Develop and review outcome-focused support plans that promote independence
• Apply a reablement-first approach in all interventions
• Collaborate with families, health professionals, and partner agencies
• Manage your own caseload effectively and meet review timelines
• Maintain accurate records using Liquid Logic case management system
• Work across multi-disciplinary teams to achieve the best outcomes for service users

? What We’re Looking For:

• Experience in social care and reablement settings
• A relevant social care qualification
• Proven experience working in a local authority or statutory/voluntary agency
• Confidence using Liquid Logic
• Strong understanding of adult social care legislation and safeguarding
• Excellent communication and interpersonal skills
• Ability to build effective partnerships and tailor support to individual needs
