About the Role: We are seeking an experienced Locum Social Worker to join Southwark's Transfer of Care Team (TOC South), based at Kings College Hospital, to support safe, timely and person-centred discharges for adults with complex needs. Working within Adult Social Care (Older Adults & Physical Disabilities), this is a hospital-based role where you will carry out complex discharge planning, including Mental Capacity Assessments, safeguarding, and multi-agency working.

  • Ability to manage complex caseloads and work collaboratively with multi-agency teams
