The post holder will manage autonomously as part of a highly motivated team of Case Managers, providing high standards of care by utilising holistic skills in assessment, planning, and implementing proactive care needs for a population with Long Term Conditions in community settings. The post holder will collaborate with Primary and Secondary care clinicians, social care, and mental health colleagues to ensure care is patient-centered and appropriate to assessed needs. The post holder will be a champion for Manchester Case Management, demonstrating clinical and professional skills in teamwork, communication, delegation, care planning/co-ordination, medicine management, health, and case management. The role requires travel between four neighbourhood sites and patients\’ homes. Manchester Case Management (MCM) is an exciting new service developed in Manchester, building on successful pilots like High Impact Primary Care (HIPC) and Active Case Management services. We seek experienced nurses to join our friendly, supportive team. You will experience a wide range of patient journeys and work closely with colleagues to promote health education, well-being, and care planning to improve patients\’ lives across South Manchester. Working across the South Manchester Locality, the Case Manager will undertake care for a defined caseload of patients, working closely with and based within one of our four integrated neighbourhood teams.
