Essential Duties and Responsibilities:
- Provides individual and group counseling services within individual scope of practice to assist clients in achieving more effective personal, social, educational, and vocational development and adjustment.
- Provides behavioral health counseling utilizing evidence-based practices and a brief intervention model.
- Available to primary care providers for behavioral health staffing and consultation.
- Collects psycho-social data about individuals and provides gathered information to providers to assist them in managing medical care.
- Performs mental health evaluations utilizing approved evaluation tools assessing amenability to mental health treatment and level of care needed.
- Develops detailed care plans for each client and documents as part of the electronic health record.
- Links clients with various community resources.
- Provides in-service training for NEW Health staff as requested.
- Participates in community outreach activities as directed.
- For staff with chemical dependency expertise: Responsible for performing chemical dependency assessments on clients using approved testing instruments and evaluating tools and determining their amenability for referral to other agencies and/or programs.
- Conducts individual and group counseling and education focusing on alcohol and drug treatment, relapse prevention, and cognitive behavioral change.
- For staff with clinical supervision training and duties: Supervises and coordinates volunteers and behavioral health/social work students as required. Facilitates peer review and chart audit process as assigned.
- Commitment to supporting a safe, respectful, equitable, and inclusive environment required.
- Travel may be required.
- Perform other duties as assigned.
Qualifications
Education/Experience: Valid state licensure as a Licensed Advanced Social Worker (or associate), Licensed Independent Clinical Social Worker (or associate), Licensed Mental Health Counselor (or associate), Licensed Clinical Professional Counselor, and/or Licensed Professional Counselor required. Two years of clinical practice with underserved populations desired.
Skills: Knowledge of available community linkage resources. Excellent oral and written communications skills required. Commitment to supporting a safe, respectful, equitable, and inclusive environment required. Computer skills required. Work requires consistent exercise of independent judgment and discretion. Valid drivers’ license and insurance required.
Physical Demands: Movement around the facility between 1/3 and 2/3rds of the day is required, as well as sitting at a workstation or patient rooms. Using hands occurs over 2/3rds of the day, while reaching with arms less than 1/3rd of the day. Communicating is required over 2/3rds of the day. Lifting to 10 lbs. occasionally is required.
