Responsibilities The Registered Nurse (RN) in Home Health provides and directs nursing care to patients in their homes, following physician prescriptions and complying with applicable laws, regulations, and agency policies. Coordinate the overall care plan with healthcare professionals and ensure continuity of care by planning and exchanging information with physicians, agency staff, patients, families, and community resources, all within a Point of Care setting. Conduct initial and/or comprehensive nursing evaluations, determine primary care focus, develop care plans within state guidelines, and document accurately and timely in collaboration with physicians. Perform comprehensive assessments at designated times and develop appropriate care plans. Ensure patients meet home health eligibility and medical necessity criteria as defined by payer sources.
