The Princess Alexandra Hospital NHS Trust (PAHT) provides a full range of general acute, outpatient and diagnostic services at The Princess Alexandra Hospital in Harlow, the Herts and Essex Hospital in Bishop’s Stortford, and St Margaret’s Hospital in Epping.

An exciting opportunity to lead our Transfer of Care team. Are you an experienced healthcare professional with extensive knowledge of the transfer of care process? We are seeking a dedicated and dynamic Transfer of Care Team Lead to oversee and manage the Transfer of Care Team at The Princess Alexandra Hospital NHS Trust (PAHT). This role is crucial in ensuring safe, timely, and efficient patient transitions from hospital to community care settings.

Main duties of the job:

  • Oversee the daily operations of the Transfer of Care Team, ensuring a seamless discharge process.
  • Work closely with the Transfer of Care Hub Lead within the Care Coordination Centre to optimise patient flow.
  • Foster strong relationships and maintain clear communication with internal stakeholders and external system partners across West Essex and East & North Hertfordshire.
  • Ensure accurate data collection and reporting to support system-wide collaboration and decision-making.
  • Lead and support a team that works across multiple hospital sites, ensuring best practices in discharge planning and care transition.

To be successful in this role, you will need:

  • Extensive experience in the transfer of care process within a healthcare setting.
  • A professional background in nursing, allied health professions (AHP), or social work.
  • Proven leadership and team management skills.
  • Excellent interpersonal and communication skills to engage effectively with multidisciplinary teams and external partners.
  • Strong analytical and problem-solving abilities to enhance service efficiency and patient outcomes.

Person specification:

Qualifications:

  • NMC or HCPC registration
  • Evidence of ongoing personal development

Experience:

  • Experience of a wide range of acute and community pathways and processes in relation to patient needs
  • Working knowledge of barriers to discharge planning
  • Experience of patient flow and bed management
