We are seeking a committed, highly motivated, enthusiastic, and skilled Band 5 Staff Nurse to join our dynamic, integrated multi-disciplinary Rapid Response Team (RRT). The RRT is one of seven accelerator sites involved in implementing and rolling out NHS England’s Urgent Community Response initiative. It is a multi-professional team comprising Nurses, Physiotherapists, Occupational Therapists, and Social Workers. The team aims to respond to crisis referrals within 2 hours, support community patients in crisis, and prevent unnecessary hospital and residential care admissions.
The team provides holistic health and social care assessments for individuals aged 18 and over, registered with a Bexley GP, and residing in the Borough of Bexley. Based at Queen Elizabeth Hospital Woolwich, the team operates 7 days a week from 8 am to 8 pm, offering urgent community response services. Currently located at Goldie Leigh - Lodge Hill, with outreach to other hospital and community sites, responding to urgent referrals from A&E, UTC, LAS 111, Acute Medical Unit, Short Stay Unit, Clinical Decision Unit, Day Care Unit, GPs, and other partners.
The post involves working within a multidisciplinary team including a Consultant Geriatrician, Senior Nurses, Occupational Therapists, and Social Services colleagues. Responsibilities include:
- Conducting comprehensive nursing assessments
- Demonstrating sound clinical reasoning and risk assessment skills
- Collaborating with community services to optimize patient care and prevent unnecessary A&E visits
The role offers opportunities to work closely with our Community Geriatrician and involves liaison with Emergency Services, community, and care home settings to facilitate appropriate referrals and care pathways. The team provides community follow-up and support for up to 5 days post-assessment. The post holder will also undertake delegated line management of junior staff, both informally and formally.

Service Details:
- Operating days: Monday to Sunday, 8:00 AM - 8:00 PM, 365 days/year
Main responsibilities include:
- Receiving and acting on urgent referrals from various services
- Partnership working to meet 4-hour breach times
- Identifying appropriate care pathways to prevent unnecessary hospital admissions
- Providing wound care, cannulation, IV therapy, and breast drain care
- Facilitating seamless multidisciplinary care through daily liaison with acute and community services
- Making timely referrals to community services
- Conducting urgent community and follow-up visits as appropriate
- Maintaining communication with GPs and participating in multidisciplinary meetings
- Engaging in service development and project work
- Participating in flexible team duties and rotations as needed
