Responsibilities of the Hospice RN/RN Case Manager include:
- Reporting directly to the Administrator, Executive Director, or Patient Care Manager Senior, maintaining primary control and professional management of each patient.
- Acting as the primary liaison between the physician, patient/family, and hospice team.
- Providing direct patient care services to hospice patients and supportive care to the patient’s family with a reasonable caseload, fostering trusting and compassionate relationships.
- Empowering patient’s caregivers through education on care for their loved one.
- Managing and collaborating with an interdisciplinary team including LPNs, CNAs, and support staff to provide comfort, peace, and dignity.
- Evaluating patient and family needs and assisting in the development and execution of the care plan.
- Maintaining all assigned patient clinical records.
- Participating in quality assessment and performance improvement programs.
About You
Requirements for Hospice Registered Nurse / RN Case Manager:
- Compassionate, kind, and empathetic towards patients and families.
- Comfortable providing care in the patient’s home environment.
- Flexible and able to maintain composure during crises.
- RN license in the requested state or eligibility to obtain one.
- At least 1 year of nursing experience in hospice, home health, ICU, geriatric, or oncology preferred.
- Additional experience in hospital settings such as Med Surg, ER, critical care, or wound care is a plus.
- Valid driver’s license and auto insurance.
- Current CPR certification.
