Are you a GPhC-registered pharmacist seeking a career in clinical services? We are recruiting a Clinical Services Pharmacist to join our multidisciplinary team at Warneford Hospital in Oxford on a 12-month fixed-term contract. Flexible working options are available, including full, part-time, and annualized hours. The Oxford Health Pharmacy & Medicines Management service employs about 80 staff, mainly clinical pharmacists and technicians, supported by dispensary and admin staff. Clinical pharmacists and medicines management technicians are based primarily on clinical sites, with supply and support functions at hubs across Oxfordshire. Services are provided to mental health inpatient and outpatient teams, including Acute adult mental health, Forensic, Learning disability, CAMHS, and community hospitals. Your role will focus on supporting the adult and older adult mental health teams at Warneford, with collaboration across Oxfordshire, Wiltshire, and Swindon, requiring travel between sites. Main duties of the job
- Develop strong clinical relationships with patients, carers, and healthcare professionals in inpatient and community settings.
- Support patients, prescribers, and healthcare professionals through MDT meetings, patient consultations, medication reviews, discharge planning, and adherence to guidance.
- Collaborate with our centralised hub pharmacist at the Clinical Pharmacy Support Unit in Oxford.
- Support development of community pharmacy services.
- Contribute to clinical effectiveness initiatives.
- Participate in the on-call pharmacist rota out of hours where applicable.
- Engage in professional development through supervision and education sessions.

Detailed job description and main responsibilities
- Optimize drug therapy through MDT participation, drug history, and care planning.
- Monitor clinical response and side effects.
- Ensure prescribing and administration adhere to evidence and policy.
- Promote multidisciplinary medication training.
- Provide medication information for discharge planning.
- Liaise with clinical teams and external agencies for needs assessment and discharge planning.
- Document interventions and records on pharmacy and trust systems.
- Provide post-discharge care and interface support.
- Offer medicines information and advice to patients, carers, and staff.
Person specification
- Aptitude and skills: Attained General Level Framework competency or equivalent experience/qualification. Awareness of quality and risk management; Driver’s License.
- Knowledge: Competence in MS Office; ability to communicate complex information effectively. Ability to analyze electronic data.
- Qualifications: Masters in Pharmacy or equivalent, GPhC registered, CPD evidence. Certificate in Psychiatric Therapeutics.
- Experience: Hospital pharmacy experience (desirable). Experience supporting mental health units. Flexible, team-oriented, self-motivated approach.
- Contractual requirements: Ability to travel between sites. Successful candidates will undergo a thorough induction. Appointment is subject to satisfactory references covering 3 years.
