Opportunity available for an Advanced Nurse Practitioner to work within an 111 Telephone Triage service based in the North West. The team operates within 111 Triage, with the role primarily responsible for delivering evidence-based nursing services as part of a multidisciplinary team. You will work autonomously, managing clinical areas such as health promotion, chronic disease management, health prevention, and well-man clinics. You will support the management team in reviewing clinical policies and procedures. You will have access to clinical supervision and support from a designated individual within the organization.
Your responsibilities include:
- Developing and implementing health promotion and wellbeing programs
- Creating and evaluating treatment plans for acute and non-acute conditions
- Supporting chronic disease management
- Identifying patients at risk of long-term conditions
- Providing routine nursing care according to clinical evidence, NICE guidelines, and the NSF
- Prescribing medications within your scope of practice and reviewing their effectiveness
The role offers flexible hours (part and full-time) with an hourly rate of £45. Benefits include:
