$30.00/hr – $35.00/hr

About the Role

MSA Home Health and Hospice, part of the Medical Services of America family, is seeking an experienced Part-Time/PRN Home Health Social Worker for our Smyth Co, Washington Co VA location.

• Provide services according to the physician’s plan of treatment and assist the team in understanding social and emotional factors affecting patient care.
• Collaborate with the interdisciplinary team to assess and support patients and families.
• Maintain accurate, timely client care records following policies.
• Utilize community and family resources to support long-term client goals and adaptation to disabilities.
• Assist in staff education as requested by the client care manager.

Job Requirements

• Bachelor’s or Master’s degree in social work, sociology, psychology, or rehabilitation counseling from an accredited 4-year college or university.
• At least one year of social work experience, preferably in home care, hospital, rehabilitation, or mental health settings.
• Current licensure to practice as a social worker in VA.
• Valid Driver’s License.
