Brook Street are delighted to be recruiting for a passionate and dedicated Care Manager to lead and inspire a team running two services in Caerphilly, for adults with learning disabilities. This rewarding opportunity is the ideal role for an experienced registered manager or progressive deputy manager ready for their next step in their career. This is a fantastic opportunity for someone who is committed to delivering outstanding care and support, and who thrives on making a real difference in people's lives.

Key Responsibilities:

  • Oversee the day-to-day running of the services, ensuring it operates smoothly and safely.
  • Lead, mentor, and develop a team of support workers to deliver person-centred care.
  • Maintain compliance with CIW regulations.
  • Create and manage individual care and support plans in collaboration with service users, families, and professionals.
  • Monitor quality standards and implement continuous improvement strategies.
  • Manage staffing levels, rota planning, and recruitment in conjunction with senior management.
  • Maintain accurate records, including care documentation, risk assessments, and audits.

About You:

  • Proven experience in a similar role within health and social care, ideally supporting people with learning disabilities or complex needs.
  • QCF Level 3 in Health and Social Care (Level 5 desirable or willingness to work towards it).
  • Strong leadership, organisational, and communication skills.
  • A compassionate and proactive approach to care management.
  • Understanding of relevant legislation, safeguarding, and person-centred care principles.
  • Driver with access to own vehicle.
