Maintain annual HHRG values at the National Standard as appropriate. Utilize HHRG values to establish visit frequency and interdisciplinary utilization. Ensure annual outcomes meet National Benchmarks. Refer patients to community resources as appropriate. Evaluate the clinical needs of the patient; develop, implement, and revise the plan of care; document accordingly. Participate in inter-disciplinary patient case conferences; evaluate the effectiveness of the care plan. Supervise and instruct the Home Health Aide plan of care every 14 days with documented evidence. Notify MD/other care providers of significant patient condition changes. Complete admission and other necessary paperwork promptly. Provide clear, concise narrative documentation reflecting skilled care, teaching, and patient/caregiver response. Obtain and clarify valid MD orders for all care provided. Plan and coordinate discharges, inform patient/MD, and document in the discharge summary; complete all discharge paperwork. Consistently deliver service excellence to patients, families, visitors, volunteers, and colleagues, reflecting VNAβs CARES values: compassion, accountability, respect, excellence, and service.
Qualifications:
- Bachelor's or Master's Degree in Physical Therapy from an accredited institution.
- Current PT license in Massachusetts, with at least one year of clinical experience and CPR certification.
- Computer skills preferred.
Purpose of Position:
Conduct comprehensive physical therapy assessments, considering physiological, psychosocial, environmental factors, and health history. Develop a plan of care to promote function and safety, and participate in policy and program development to enhance patient care.
Schedule Details:
37.5 hours/week, various shifts including some holidays and weekends. Area: Upper Cape. Sign-on Bonus Eligible!
