Wirral Falls Prevention Service Bromborough

Hours: Temporary, up to 22.5 hours per week

Salary: £24,225 per annum (Pro rata)

This post is for a band 6 equivalent Occupational Therapist or Physiotherapist to join Medequip’s Wirral Falls Prevention Service. About the role

As the Falls Practitioner for Medequip Assistive Technology Ltd, you will provide a high-quality service to pre-frail older adults who have fallen or are at risk of falls. Through multi-factorial falls risk assessments and by providing Public Health advice and interventions to promote an active and healthy lifestyle in order to prevent falls. Your day-to-day responsibilities will include, but not be limited to:

  • To undertake a specialised comprehensive multifactorial falls risk assessment of people at risk of falling and to provide an accurate diagnosis/impression of their falls risk factors.
  • Formulate intervention plans and recommend best course of action.
  • Where appropriate assess for and provide low level community equipment and walking aids including assessment for grab rails.
  • To refer more complex cases to specialised therapy services.
  • Refer patients to other agencies and professionals.
  • Use a range of verbal and non-verbal communication tools to communicate effectively with patients to provide falls prevention advice and care plans.
  • Work within professional guidelines of Chartered Society of Physiotherapy (CSP), College of Occupational Therapy or NMC guidelines as appropriate and to have a good working knowledge of national and local standards and monitor own quality of practice.
  • To be responsible for maintaining accurate and comprehensive clinical records.
  • To assist in the training and development and supervision of technical instructors and students.
  • Maintain and develop current knowledge of evidence-based practice.
  • Contribute to the Clinical Governance agenda and participate in audits as required by the Service.
  • Ensure that your own practice and that of staff under your supervision meet the required professional standards of practice.
  • Deputise for the Team Leader in their absence, taking responsibility for the operational management of the team.
  • To be responsible for organising and planning own caseload to meet service and patient priorities, readjusting plans as situations change/arise.

Qualifications and experience required

  • Diploma/ Degree in Physiotherapy / Occupational Therapy is essential.
  • HCPC registered.
  • Driving license and access to own vehicle is essential.
  • This is a Band 6 equivalent job role and experience is preferred.
