An exciting opportunity has arisen for a Dementia Liaison Nurse based in Wirral, covering a 7-day service. Although this position is based in Arrowe Park Hospital, the role is a community-based position.

The Dementia Liaison nurse post is an innovative and creative solution to issues faced by people living with dementia and their carers. The liaison nurse will aim to enhance the care of people with dementia, while supporting carers and other professionals. The role also aims to increase primary care’s awareness and capacity to understand and cope with dementia in the future.

The successful applicant will be expected to work across Urgent Care Mental Health First Response Services, including Crisis Resolution Home Treatment Teams, Liaison Services, Street Triage Services, Liaison and Diversion Services, and the Crisis Line.

Main duties include working autonomously to deliver specialist assessment and interventions in a non-mental health setting, including care planning and working collaboratively with other disciplines and agencies to provide an effective and seamless service.
