About the Role As a Social Worker in our Adult Social Care team, you\’ll play a crucial role in assessing and addressing the diverse needs of our community, navigating the varied challenges our vibrant city provides. Your responsibilities will include: Conducting assessments and developing tailored care plans for service users and carers, ensuring their unique needs and priorities are addressed. Advocating for choice and control , empowering individuals to make decisions and manage risks, ensuring services are organised in alignment with their preferences and needs. Upholding professional standards continuously as our service evolves. You\’ll monitor changing needs, develop support plans, utilise resources, and ensure Social Care remains future-ready through creative use of assistive technology.
