ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Perform comprehensive patient assessments to determine functional deficit and potential and establish a therapeutic program.
  • Develop a therapy plan of care to include goals and discharge plans.
  • Document, within time frames, observations, patient's response to treatment, treatment programs, degree of progress and notable changes in patient condition.
  • Complete all documentation required and in accordance with state and federal regulations and policy.
  • Demonstrate administration of objective, standardized tests and measures and performance of evidence-based practice in documentation.
  • Communicate and coordinate patient care with other members of the patient's health care team.
  • Attend employee meetings and patient conferences.
  • Supervise physical therapy assistants in accordance with regulations and policy.
  • Provide patient and family teaching related to the patient's physical therapy program and patient/family engagement in goal achievement.
  • Assist the patient and/or family to procure needed equipment or supplies.

POSITION REQUIREMENTS:

  • Graduate of an accredited Physical Therapy program
  • Licensed as a PT in Massachusetts
  • Home care experience strongly preferred.
  • Current CPR required.
  • Current State of Residence driver's license and reliable transportation.
