BAYADA Home Health Care is seeking an experienced Physical Therapist (PT) for a Part-Time or Per Diem opportunity to perform home health visits for adults and seniors in Senior Living Communities across Cumberland and Franklin Counties.
Requirements include at least one year of clinical experience as a licensed Physical Therapist. As a home care PT, you will be an essential part of a multidisciplinary healthcare team providing rehabilitative care and skilled nursing, enabling clients to remain in their homes. Responsibilities include:
- Conduct home visits within designated areas.
- Perform diagnostic tests such as mobility, joint range of motion, transfer status, ambulation patterns, muscle strength and endurance, balance, and safety assessments.
- Implement wound prevention and repair interventions using evidence-based dressings and topical agents.
- Create and update individualized care plans, including manual therapy, gait training, and balance interventions.
- Assess and revise care plans regularly, collaborating with nursing staff as needed.
- Educate clients and their families on rehabilitative activities to promote health, safety, and independence.
- Document observations and interventions accurately using electronic tablets.
Qualifications include:
- Active Pennsylvania PT license.
- Minimum of one year recent experience as a PT.
- Graduation from an APTA-accredited program, evidenced by transcript or diploma.
- Ability to work independently and manage time effectively.
- Strong interpersonal, organizational, and problem-solving skills.
- Proficiency with computers; EMR experience preferred.
