The Case Manager - Medical Outreach role is responsible for providing case management services and achieving positive intervention outcomes for assigned clients. They work with ongoing patient caseloads, addressing medically and socially challenging issues, and overcoming barriers to health.
Required Minimum Knowledge, Skills, and Abilities (KSAs)
- Education: Bachelor's Degree preferred, with a specialization in Psychology, Behavioral Health, Social Work, Special Education, Counseling, Rehabilitation, Nursing, or a closely related field.
- License/Certifications: Current applicable state license as a Licensed Social Worker preferred. Valid driver's license and reliable vehicle required.
- Experience: With a bachelor's degree: Two years of experience in a community health setting, including one year of case management. Without a bachelor's degree: Three years of relevant healthcare experience, including 6 months of case management. Experience collaborating with state and federal agencies, community agencies, municipal, and public programs is preferred.
