Salary: $82,000 - $115,000 per year

Responsibilities
The RN in Home Health provides and directs nursing care to patients in their homes, as prescribed by the physician and in compliance with applicable laws, regulations, and policies. The RN coordinates the total plan of care with other healthcare professionals and helps maintain continuity of care by planning and exchanging information with physicians, agency staff, patients, families, and community resources, all within a Point of Care setting. Provides clinical services within the scope of practice, as defined by state laws, in accordance with the care plan and in coordination with the healthcare team. Conducts initial and/or comprehensive nursing evaluations, determines the primary focus of care, develops the care plan within state-specific guidelines, and documents accurately and timely. Regularly evaluates and re-evaluates the patient's nursing needs, at least every 60 days or as conditions change. Performs comprehensive assessments at designated times and develops appropriate plans of care in collaboration with physicians. Ensures patients meet home health eligibility and medical necessity guidelines as defined by the payer. Initiates, develops, implements, and revises the care plan collaboratively with physicians and other healthcare professionals.
Education and Experience License Requirements
- Current RN licensure in Washington State
- Current CPR certification
- Valid driverβs license, vehicle insurance, and access to reliable transportation
