We are seeking a community mental health nurse practitioner with excellent organizational and communication skills, and an interest or experience in facilitating groups and workshops. The successful candidate will join our Bexley Community Mental Health and Wellbeing Hub, a service developed through the Community Mental Health Transformation Programme, delivered by a partnership of clinical staff from Oxleas, Bexley Mind, Primary Care, Bexley LA, voluntary, third-party, and local partners.

The hub supports individuals managing mental health issues along with related challenges such as employment, relationships, housing, finances, and community engagement. Referrals mainly come from primary care sources, including GPs and the London Borough of Bexley.

The role involves conducting comprehensive telephone and face-to-face assessments to identify patient needs, managing a caseload for brief interventions, and working within a supportive, multidisciplinary team. The ideal candidate will be registered with the NMC, with significant post-qualification experience in a community, primary care, or single point of access setting.
