We are looking for a Paediatric Diabetes Specialist Nurse to join our excellent paediatric diabetes team in Rotherham. Our service cares for children and young people with diabetes (aged 0-19 years) in acute, outpatient, and community settings. We aim to improve outcomes through a cohesive multi-disciplinary team focused on patient care. The post holder will utilize their skills in clinical expertise, leadership, education, and management to ensure a seamless service. They will work closely with hospital and community teams to provide coordinated, quality care. Additionally, there will be collaboration with General Practice, schools, and colleges in Rotherham. The role involves supporting the development and delivery of diabetes education to patients, carers, and healthcare professionals, and maintaining an effective, patient-focused service that is sensitive to the needs of users and providers.
Main duties of the job:
- Deliver high-quality nursing care to people with diabetes within the framework of the PDSN Team.
