To work as part of a multi-disciplinary team, providing a high standard of holistic care and support to children and young people from 0-18, and their families. To be responsible for the assessment, planning and delivering of evidence-based nursing care to children and young people with life limiting conditions, and supporting their families To work actively towards achieving the skills to co-ordinate a shift, ensuring that all care is delivered in accordance with HDH policies and procedures To contribute to the sharing of knowledge and training within the organisation in conjunction with the learning and development team. To support 24 7 care service consisting of a variety of shifts, including earlies, days, lates, nights and weekends.
