Provide the leaving care personal advisor role for eligible, relevant, and former relevant young people as required by the Children (Leaving Care) Act 2000 and volume 3 of the Children Act 2010.

Responsibilities include:

  • Producing and reviewing the pathway plan and making regular 2-monthly visits to care leavers.
  • Assisting young people in assessing their needs before transferring to the Leaving Care Team, and engaging in reviews and discussions of the pathway plan for 16/17-year-olds with CLAFH colleagues.
  • Working within the Redbridge Leaving Care policies and procedures.
  • Maintaining accurate statutory and administrative records, including the use of IT systems.
  • Ensuring confidentiality and adhering to data protection guidelines.
  • Supporting young people and their social workers as they prepare to leave care at age 16 by providing advice, group work sessions, and focusing on developing independent living skills, careers, educational opportunities, and accommodation options.
  • Providing ongoing support to young people over 18 through individual and group sessions aimed at improving life skills, reducing social isolation, and encouraging peer support.
  • Maintaining knowledge of accommodation providers and working with young people to ensure contact and security of licences or tenancies.
  • Assisting with applications and facilitating smooth transitions to permanent council accommodation.
  • Developing and maintaining positive relationships with agencies such as Connexions, careers and training providers, colleges, local employers, and the community to promote access to employment, training, education, and leisure activities.
  • Involving young people in planning, delivering, and evaluating services.
  • Encouraging participation in the Children in Care council and consultation systems within the borough.
  • Assessing young people's financial needs, organizing payments, benefits, or grants from educational or charitable sources, and supporting the management of leaving care grants.
  • Participating in staff training and development programs organized by the team or department.
