Job Description:
The role is the first point of contact for potential program participants in Support Coordination to scope both the services required and provide a comprehensive and timely service package for individuals to consider. This role will work closely with NDIS clients to ensure they receive a timely and responsive service offering in line with their support plan. This position is a 'risk assessed role' and is subject to the NDIS worker 'no card' 'no start' rule.
Key Responsibilities include:
- Develop strong communication and liaison with all current and potential NDIS clients to ensure services offered and implemented are responsive and in line with requirements.
- Establish, support and design plans to address the identified needs of the participant in line with NDIS plans, individual requirements and NDIA guidelines.
- Explore, identify, develop, strengthen and facilitate formal and informal networks with individuals and their families to maximise opportunities for participation, independence and inclusion in the broader community.
- Identify new opportunities within Support Coordination.
- Participate in Expos, networking and forums to raise Centacare's profile.
- Contribute to a positive, collaborative team that models best practice.
Centacare North Queensland is a registered NDIS provider and this position is subject to the NDIS worker 'no card' 'no start' rule. This position is classified as a 'risk assessed role' under Part 1, section 5 of the National Disability Insurance Scheme (Practice Standards—Worker Screening) Rules 2018 and in accordance with Queensland legislation, an NDIS Worker Screening must be obtained prior to commencing work in the role.
Desired Skills and Experience:
- A recognised tertiary degree in either Behavioural or Social Science or equivalent OR five years post-graduate experience in the disability sector OR experience working with families and people with disabilities in a support coordination role.
- Highly developed analytical, evaluation and planning skills in the disability sector, including an understanding of the NDIS guidelines.
- Excellent written and verbal communication skills as well as strong analytical skills.
- Demonstrated commitment to 'inclusion' principles and personalised support practices in the disability sector.
- Highly developed IT and report writing experience.
- Drivers’ Licence.
- A current Blue Card and NDIS worker screening clearance.
