To be responsible for maintaining an effective medicine management system within the practice, including cost-effective prescribing, providing advice and support for both patients and colleagues.
To contribute to proactive patient care through supporting the practice processes for managing long term conditions, high risk drugs, MHRA alerts and medicines optimisation projects.
You will support for the provision of same-day consultations as part of the practices multidisciplinary on the day team. The post holder will assess, diagnose, treat, refer or direct to other services patients who attend surgery, request telephone consultations and online consultations, or require home visits relating to unscheduled minor conditions, injuries or acute illness within your scope of competence.
The post-holder will also support the multi-disciplinary team, in line with the strategic objectives of the practice, to improve patient safety, clinical outcomes and practice cost effectiveness tailored to individual patient needs
