Purpose of the Job:

The post holder will be part of Adult Services, working to assess the social care needs of people in accordance with the wellbeing principles of the Care Act, offering a personalised and strengths-based approach.

They will promote personalised support, facilitate service user choice and control, manage a caseload, conduct assessments in partnership with service users, carers, and multidisciplinary partners, and identify outcomes and asset-based goals to develop support plans. The role involves assessing and managing risk, operating within performance and budget frameworks, and ensuring quality assurance and safeguarding procedures are followed.

Knowledge of Adult Social Care:

• Understanding of the Care Act, Mental Capacity Act / DOLS
• Knowledge of London Multi-Agency Adult Safeguarding policies and related legislation
• Awareness of best practices and integration initiatives in health and care
• Understanding of needs of Adults at Risk and those with long-term conditions and complex needs
• Knowledge of assessment and support planning for diverse communities, focusing on independence and strengths-based solutions

Technical Skills:

• Proficiency with IT tools including databases, word processing, email, and spreadsheets

Qualifications:

• UK state-registered degree in Social Work, DipSW, or CQSW
• Current Enhanced Criminal Records Bureau assessment
