The post holder will be part of Adult Services, working to assess the social care needs of individuals in accordance with the wellbeing principles of the Care Act. They will offer a personalised, strengths-based approach, managing a caseload, conducting assessments in partnership with service users, carers, and multidisciplinary teams.
The role involves identifying outcomes, supporting individuals to meet their long-term needs and aspirations through asset-based goals, and developing outcome-focused support plans. The post holder will also assess and manage risks, ensuring adherence to quality assurance and safeguarding procedures.
Knowledge and Skills
- Understanding of the Care Act, Mental Capacity Act, and DOLS.
- Knowledge of London Multi-Agency Adult Safeguarding policies and related legislation, including Making Safeguarding Personal.
- Awareness of best practices and initiatives across health and care sectors.
- Understanding of the needs of Adults at Risk, including those with long-term conditions and complex needs.
- Experience in assessment and support planning within diverse community settings, promoting independence from a strengths-based perspective.
Technical Skills
- Proficiency with IT tools such as databases, word processing, email, and spreadsheets.
Qualifications
- UK state-registered degree in Social Work, DipSW, or CQSW.
- Current Enhanced Criminal Records Bureau assessment.
