Project Description: The project is funded through The National Lottery Community Fund and operates across Swansea NPT working with people who have multiple overlapping and unmet needs. We are a team of professionals from different organisations that come together to provide Assessments, Interventions and Support together in one place which we call the Swansea NPT 360, with the aim of engaging vulnerable individuals in support services and signposting to relevant support.
Purpose of Role: To provide housing-related support to vulnerable individuals—such as those experiencing homelessness, mental health challenges, or substance misuse—within a collaborative MDT setting. The goal is to promote independence, tenancy sustainment, and improved wellbeing. This role will utilise an assertive outreach approach to build engagement and trust within the project.
- Develop and implement person-centred support plans in collaboration with service users and MDT colleagues.
- Provide practical support with housing applications, tenancy agreements, budgeting, and daily living skills.
- Liaise with health professionals, social workers, and other agencies to coordinate holistic support.
- Conduct risk assessments and contribute to safeguarding processes.
- Support clients through crisis situations and transitions, including hospital discharge or move-on accommodation.
- Maintain accurate records and contribute to service reviews and audits.
This role is subject to an Enhanced DBS disclosure.
