The post holder will provide clinical expertise and operational management for the Virtual Wards (Hospital @Home) Model within Birmingham Community Healthcare (BCHC) NHS Foundation Trust. The role involves working across system-wide settings, including community and inpatient units, and in-reach into secondary care, to identify and assess patients, prevent avoidable hospital admissions, and facilitate early supportive discharges.

This work will be collaborative, involving Secondary and Primary Care, Community Services, Urgent Care Response, Early Intervention Team (EICT), and Specialist Services. The post holder will attend integrated multidisciplinary teams (MDTs) to offer advice, support, and data to support the Virtual Ward (Hospital @Home) Model.

Under supervision, the post holder will provide direct clinical care, expert support and advice, and manage aspects of the service within teams. Responsibilities include assessing, diagnosing, and managing a highly specialist adult patient caseload in various settings, including assessment, diagnosis, treatment planning, therapy, and discharge planning.
