Position Overview: The Case Manager will provide case management services and work with the clinical staff to coordinate care and assist with treatment plans for clients.
Responsibilities:
- Participate in assessments and development of service plans.
- Implement services according to the individualized service plan (ISP).
- Collaborate with patients and their families to execute effective service plans, explaining clinical options.
- Document patient progress and maintain comprehensive clinical records.
- Coordinate care with the clinical team, providers, and community resources to ensure plan implementation.
- Assist in maintaining, monitoring, and modifying covered behavioral health services.
- Identify and seek resources beyond covered services to meet patient needs.
- Ensure referrals are made and coordinated with service providers; update service plans monthly.
Minimum Qualifications:
- High School Diploma and at least one year of experience in Behavioral Health or Psychiatry.
- Bachelor's Degree in a social work-related field preferred.
- Knowledge of healthcare, detoxification, addiction, co-occurring disorders, DSM criteria, and terminology.
- Strong reading, writing, speaking, and listening skills; proficient with Microsoft Office.
Competencies: Includes job knowledge, communication, problem-solving, decision-making, collaboration, and adaptability to work environment and physical demands.
This job operates in a professional office environment with standard equipment and moderate noise levels. Travel is primarily local with some overnight travel expected.
