Job Description
The post of a social worker involves supporting the discharge of residents from the borough to local acute hospitals, out-of-borough hospitals, and the local Rehabilitation Unit. The team operates in accordance with the Discharge to Assess model of care to coordinate services that facilitate the recommended discharge pathways.
The ideal candidate should have previous experience in adult social services, with a strong interest in hospital discharges. They should possess excellent skills in multidisciplinary work, assessment, and undertaking Mental Capacity Assessments, along with a thorough knowledge of relevant social care legislation, safeguarding procedures, and the continuing healthcare process.
