Description The Navigator explores treatment recovery options with enrollees, helps clarify goals and strategies, provides education and resources, and assists enrollees in accessing treatment and community supports. What You Will Doing to Make a Difference… Closely collaborates with the enrollees to develop an individualized plan based on identified needs and goals Assist enrollees with care coordination and scheduling medical appointments Implement recovery oriented, motivational interviewing, harm reduction, and strengthen-based rehabilitation practices.
