Certified Nursing Assistants with Hospice Experience (Healthcare)

Traditions Home Health Services is seeking compassionate, skilled, reliable, and experienced Hospice-trained Certified Nursing Assistants to assist clients on Hospice residing in the Plymouth area.

Available Shifts:

• Morning: 8am – 12pm
• Evening: 5pm – 10pm

Job Responsibilities:

• Assist with daily living activities such as toileting, incontinence care, bathing, dressing, grooming, nail care, oral hygiene, and skin integrity maintenance.
• Monitor vital signs, facilitate daily exercises, and perform range of motion exercises as per the care plan.
• Prepare healthy meals and ensure adequate hydration according to the care plan.
• Follow infection control protocols.
• Administer medication reminders and assist with eye drops, ointments, creams, and ear drops.
• Perform housekeeping tasks to maintain a safe and clean environment.
• Engage clients in stimulating activities to promote joy.
• Provide transportation for outings, errands, and appointments.
• Proficient in dementia care.
• Document shift activities digitally using a smartphone.
• Report concerns promptly to the Office Support Team.
