The CCS Youth Coordinator provides a coordinated and comprehensive array of recovery support services, treatment, and psychosocial rehabilitation services that assist individuals to utilize professional, community, and natural supports to address their needs. Actively coordinates the process of service planning and delivery, as well as the traditional case management function of helping the client to access services necessary for the recovery of AODA related and mental health conditions. The program is person-centered and uses client-directed service plans to describe the individualized services that will support the client to achieve their recovery goals. Services are provided by teams of professionals, peer specialists, and natural supports, all coordinated by a CCS service facilitator. CCS is a community-based program in which the majority of services are provided in clients’ homes and communities. Comprehensive Community Services (CCS) is a recovery-focused, integrated behavioral health program for adults with severe mental illness and/or substance use disorders and children with severe emotional disturbance. CCS is unique for its inclusion of both children and adults and its focus on other physical illness and impact on multiple system use. ESSENTIAL FUNCTIONS: Adheres to St. Charles Youth & Family Services Policies and Procedures. Provide care coordination services to each young adult throughout the duration of enrollment in CCS services. Maintains a caseload of 12 – 15 clients. Accepts referrals of eligible consumers and initiates the process to assess strengths and needs. Set up teams made of formal and informal resources that make up the support network. Facilitate initial team meetings to develop a care plan based on strengths, values and preferences that will assist the young adult to reach favorable and measurable outcomes. Educate young adults on how to utilize community supports, both formal and informal. Assist team in establishing measurable consumer outcomes and help develop a plan to reduce any system barriers among community-based, Child Welfare or other agencies. Document plans, collect outcome data, prepare all documentation according to program-required time frames and maintain weekly timesheets. Participate in administrative and collaborative meetings and activities. Attend training and ongoing staff development opportunities to enhance skills. Other duties as assigned. MINIMUM KNOWLEDGE, SKILLS AND ABILITIES REQUIRED: Bachelor degree in Social Work, or related field required. Knowledge base of individual and family issues and dynamics. Ability to prepare formal reports. Ability to display cultural competence by responding respectfully and effectively to people of all cultures, languages, classes, races, ethnic backgrounds, religions and other diversity factors in a manner that recognizes, affirms and values the worth of individuals, families and communities and protects and preserves the dignity of each. Strong verbal and written communication skills. Valid Wisconsin driver’s license and liability insurance. Ability to use vehicles for work-related duties according to the agency’s insurance liability standards. Ability to accurately perform record keeping and documentation entries. Working knowledge of computer applications preferred. WORKING CONDITIONS: General Office Environment Ability to lift up to 20 lbs. Community and in-home setting during client visits.
