Job Accountabilities

1. Meet reablement goals/outcomes as outlined in the care plan.
2. Identify and support additional goals that may enhance quality of life and independence.
3. Provide feedback on progress and goals to the Reablement and Assessment Officer.
4. Maintain clear and timely communication regarding individual progress.
5. Collaborate effectively within a small team to ensure consistent support and address concerns.
