Job Accountabilities
- Meet reablement goals/outcomes as outlined in the care plan.
- Identify and support additional goals that may enhance quality of life and independence.
- Provide feedback on progress and goals to the Reablement and Assessment Officer.
- Maintain clear and timely communication regarding individual progress.
- Collaborate effectively within a small team to ensure consistent support and address concerns.
