Vitalis is recruiting an experienced Senior Social Worker to work in a Children with Disabilities Team in Birmingham.

The Senior Practitioner will be responsible for a caseload of children and young people with severe and profound levels of disability. Responsibilities include completing Child and Family Assessments, supporting through Child in Need Plans, CP Plans, and CIC plans. The role involves undertaking visits to children, young people, parents, and carers; conducting reviews; chairing and contributing to multi-agency meetings; and ensuring Child Plans are SMART, meet the needs, and promote the best possible outcomes for the child or young person and their family.
