Contract role

Vitalis is recruiting an experienced Senior Social Worker to work in a Children with Disabilities Team in Birmingham. The Senior Practitioner will be responsible for a caseload of children and young people with severe and profound levels of disability. Responsibilities include:

  • Completing Child and Family Assessments
  • Supporting through Child in Need Plans, CP Plans, and CIC plans
  • Undertaking visits to children, young people, parents, and carers
  • Conducting reviews
  • Chairing and contributing to multi-agency meetings
  • Ensuring Child Plans are SMART, meet the needs, and promote the best possible outcomes for the child or young person and their family
