SALARY RANGE: 25.71 - 38.56 HOURLY
General Summary: Provides professional nursing care to patients within Physician’s Practice. Professional nursing practices, requiring specialized knowledge, judgment, and skill are performed in accordance with practice policy, procedure, and philosophy. Manages the overall quality and scope of care of patients in the urology program in conjunction with the urology care team. Navigate and collaborate with other members of the patient care team to direct and evaluate delivery of urologic patient care across all delivery sites. Collaborate with urology providers, oncologists, primary care providers, and other subspecialties on the provision of comprehensive care plans.
Responsibilities:
- Demonstrate the ability to provide the following patient care services:
- Perform triage services for patients
- Prepare injections as required and according to treatment plans designed by the physician
- Review medication refills with patient via telephone. Ensure that the patient is periodically assessed in the office by the physician to determine therapeutic effect and patient compliance. Adjust patient medications as needed
- Assist with minor medical/surgical procedures in the practice setting
- Demonstrate the ability to recognize abnormal lab results. Required to review lab reports with patients in the absence of the provider.
- Coordinate further testing and evaluation with the physician
- Identify the need for patient education. Advise patients on their treatment plan
- Maintain treatment records/files for each patient. Complete patient encounter form at the conclusion of each service performed
- Return patient phone calls regarding testing and/or treatment plan
- Maintain patient confidentiality; comply with HIPAA and compliance guidelines established for the practice
- Ensure compliance with Joint Commission regulations and other safety requirements; this includes disposal of used sharps and contaminated materials, as well as safe storage of medications
- Re-order supplies as required and within budgetary limitations
- Submit requests for capital equipment during the annual budget development process
- Maintain workstation cleanliness
- Maintain instruments and equipment; ensure all are sterilized, arranged, and ready for use
- Evaluate his/her own nursing practice in relation to professional practice standards, relevant statutes, and regulations
- Engage in an annual self-assessment, identifying areas of strength and areas requiring professional development
- Seek and reflect on constructive feedback from other members of the healthcare team
- Takes action to achieve performance goals
- Participate in ongoing educational activities to acquire and validate knowledge i.e. CPR, CLIA training programs
- Seek experiences that reflect current clinical competencies and new clinical information
- Maintain professional affiliations
- Attend regularly scheduled staff meeting as required by the practice
- Perform all other tasks and projects assigned by the Practice Manager or Physician
- May be required to act as backup for other members of the practice staff.

Required Education, Credential(s) and Experience:
Education: Certificate Program Licensed Practical Nurse; Or equivalent credit hours in an RN program.
Credential(s): Licensed Practical Nurse; Basic Life Support; OPN-CG within 36 months of hire, if not already obtained.
Experience: Required: 2 years nursing, care management / patient navigation, or related physician practice experience.
