As a âHealth Improvement Practitionerâ you will work in our BaNES Integrated Neighbourhood team and be a source of specialist advice and support for our XPert diabetes service for some of the most vulnerable people living in Bath and Northeast Somerset. Although your main focus will be supporting people to manage their diabetes diagnosis, you will at times to a lesser extent, support people with weight management, healthier cooking and physical activity in-line with our person-centered approach to improving peoplesâ overall health and wellbeing. This work will be carried out on a one-to-one and group basis within different settings throughout Bath and Northeast Somerset.
As a Health Improvement Practitioner, you will work 22.5 hours per week where your working hours will be spread over 3 days FTC until March 31st 2026. A typical working week is Monday to Friday but may include some evenings and weekend.
Main Responsibility: As a Health Improvement Practitioner within HCRG Care Group youâll join a highly skilled team who are focused on service excellence. This role focuses on the design, delivery, and evaluation of the Xpert diabetes programme across Bath and North East Somerset. Youâll provide structured support to individuals living with diabetes through a range of settings including group sessions, one-to-one support, telephone and digital engagement. A key part of the role involves delivering training to health professionals and promoting the programme to increase awareness and accessibility. Youâll work closely with local services, particularly secondary care, to ensure continuity of care for those with diabetes. Thereâs a strong focus on supporting individuals in areas of health inequality, contributing to wider health initiatives like weight management, nutrition, smoking cessation, and physical activity.
Key Responsibilities:
- Deliver and monitor the Xpert diabetes programme in various formats and settings
- Maintain a busy caseload and provide tailored support to individuals
- Train and support other professionals in diabetes care and related interventions
- Develop and share health promotion materials and resources
- Work in partnership with local services to improve access and reduce inequalities
- Support wider public health campaigns and contribute to related health programmes
- Assist with programme evaluation, data collection, and reporting outcomes
- Promote health messages through local campaigns and media where appropriate
- Supervise volunteers and offer peer support to colleagues when required
Additional Information: Based at Kempthorne House, with travel across Bath and North East Somerset. Hybrid working is expected, with office, community, and remote components. Some evening and weekend work may be required. No budget responsibility, but may supervise volunteers and support networks. Regular interaction with a wide range of health and community partners.
This role plays a vital part in delivering public health services aligned with both local and national priorities, with a primary focus on diabetes and a broader contribution to community wellbeing.

- Qualifications & Experience: Degree or equivalent in Mental Health, Wellbeing, Healthcare, Customer Service, or a related field
- Proven experience leading and motivating teams
- Confident in managing office systems, procedures, and digital tools (Word, Spreadsheets, Teams)

- Experience in monitoring, evaluation, and project coordination
