Youâll be part of a dynamic office-based team based at Peasedown St John that harnesses your clinical expertise to guide and support patients. Through pioneering use of remote technology we provide acute care from home.
As a Triage Clinician you will be joining our Triage team of Nurses, Occupational Therapists and Care Coordinators. We use cutting-edge telephone technology to effectively triage and manage referrals, providing urgent, immediate, and decisive responses tailored to patient needs. Youâll also collaborate with our Virtual Ward and Urgent Care response team, leading the charge in innovative patient care.

Main Responsibility: As a Triage clinician you will be a vital part of keeping hospital admissions down but ensuring patients are put-first â but all from the comfort of an office setting! You will:
- Gather patient information and make efficient referrals using telephone, electronic systems, or emails.
- Lead the assessment of a patientâs condition.
- Identify the most appropriate care pathway.
- Use clinical reasoning and knowledge of evidenced-based practice to inform intervention.
- Understand and apply knowledge of clinical systems to enable appropriate documentation of clinical records.
- Act as a clinical resource for Care Coordinators and Care Journey Coordinators.
- Maintain accurate systematic documentation of Patient/Health Professional interactions/advice provided.
The Ideal Candidate: If youâre an Allied Health Professional with experience in urgent or community care, we want to hear from you! Weâre seeking individuals ready to step away from physically and emotionally demanding patient-facing roles. Depending on experience this post could be offered at band 6 or band 5 as a developmental post. This is a great opportunity for an experienced band 5 to consolidate knowledge and skills and develop triage skills.
You should be:
- Registered with HCPC Band 5 (min. 3 years experience) to join in a developmental post or Band 6.
- Have a passion for multidisciplinary collaboration.
