As a Assistant Practitioner Band 4you will join our innovative and forward-thinking team of Community Nurses and Healthcare Assistants, where your growth and development truly matter. As community services evolve, we remain committed to a responsive, patient-centred approach. You’ll collaborate closely with a range of services and care for a diverse group of patients, helping deliver the right care, in the right place, at the right time.

We prioritise flexible, agile working while maintaining high-quality care, offering a 7-day service from 08:00 to 22:00. This role allows you to rotate across our community teams, including urgent response, community nursing, and Hospital at Home giving you the chance to broaden your experience and clinical skills.

Main Responsibility

As a Assistant Practitioner Band 4you will be responsible for:

  • Adheres to NMC Standards of Proficiency for Nursing Associates, maintaining professional and safe practice.
  • Manages an allocated caseload, working collaboratively within a multi-skilled team to plan and prioritise care effectively.
  • Utilises appropriate assessment tools to evaluate patient needs and risks, including safeguarding and mental health concerns.
  • Offers health promotion advice to support patient wellbeing, covering areas like diet, smoking, and alcohol use.
  • Recognises when to escalate risks and follows local and national protocols to ensure high standards of care.
  • Confidently uses digital systems to access, record, and share clinical information within interdisciplinary teams.
  • Balances workload efficiently and delegates aspects of care safely when appropriate.
  • Supports care delivery for individuals with both routine and complex needs.

The Ideal Candidate

Essential:

  • Registered Nursing Associate with NMC (or working toward required qualifications, including the foundation degree).
  • Holds GCSEs (A–C) in English and Maths or equivalent Level 2 qualifications.
  • Experienced in healthcare settings, working collaboratively under supervision.
  • Confident managing varying workloads and responding to patient needs sensitively.
  • Understands the limits of own role and actively engages in ongoing professional development.
  • Aware of how the Nursing Associate role fits within the wider team and contributes to service improvements.
  • Committed to upholding NMC Code and professional standards.
  • Knows when to seek guidance and escalate concerns appropriately.
  • Communicates clearly and compassionately with patients, families, and colleagues.
  • Proficient in IT and clinical systems.
  • Competent in safe medication administration in line with Trust policy.
  • Supports and guides trainees, students, and healthcare assistants when required.
  • Enthusiastic about delivering high-quality, safe care.
  • Works independently with a proactive and professional approach.

Desirable:

  • The Care Certificate.
  • Level 3 Health related qualification
  • Previous experience of working within an Acute health / care setting.
  • Evidence of involvement in support / development of less experienced staff.
  • Demonstrates knowledge of clinical audit and improvement methodology.
  • Able to provide a portfolio of learning in relation to knowledge & experience acquired whilst completing Trainee Nursing Associate programme.
  • Evidence of recent work-based learning or self-directed learning.

Other requirements:

  • Willingness to work across different Trust areas as required.
  • Valid UK driving license with access to a vehicle for work purposes.
