The Role: The Home Support Coordinator supervises a dispersed team of Home Support Workers providing reablement services across the borough. The role involves working with internal and external partners to support individuals in retaining, regaining, and developing skills for managing their lives and environments.
Responsibilities include safe recruitment, allocation, and monitoring of workers' work quality, developing risk management plans, contributing to care plans and reviews, and safeguarding individuals from harm and abuse.
About You: You will assist with screening and allocation of support packages using computer-based systems, require physical fitness for tasks like lifting and standing, and ensure high standards of service delivery to support vulnerable adults with rehabilitative and mental health needs. Knowledge of relevant legislation and current developments affecting adults and older adults is essential. Punctuality and flexibility to work over 365 days, including evenings, weekends, and bank holidays, are required.
