Cheshire and Wirral Partnership Foundation Trust are seeking applications for a Staff Grade doctor in Early Intervention & Older Adults. The post holder will be a core member of the Early Intervention team in Ellesmere Port and the Older Adult Team in West Cheshire. The post holder will join an integrated service based at Cherrybank Resource Centre and Upton Lea. The team includes a Consultant, CPNs, OTs, SWs, admin staff, Psychology, and a Team Manager. There is also an Advanced Nurse Practitioner (non-medical prescriber) working across the AMH teams, with potential for core trainee depending on the applicant’s educational supervisor status and trainee needs. Responsibilities include: Assessing, diagnosing, reviewing, and formulating treatment and risk management plans for referrals to the Ellesmere port and Neston CMHT. Implementation of the Mental Health Act. Care planning, treatment formulation, and guidance on evidence-based treatment and effectiveness. Providing case discussion opportunities and supervision to team members, including the Advanced Nurse Practitioner, memory nurses, and trainees if applicable. Clinical leadership of the team. Participation in weekly team meetings and monthly team business meetings. Engagement in multi-disciplinary, multi-agency, and partnership working with services such as: Crisis Resolution/Home Treatment Service Adult Community Mental Health Services Integrated Physical Health Care Teams Care Home GP Leads Primary Care Services Psychological Therapy Services Cheshire and Wirral Partnership provides health and care services including mental health, learning disability, community physical health, and all-age disability care, including three GP surgeries.
