Exciting Opportunity for Community Nurses An exciting opportunity has arisen for aspiring individuals to develop their careers in the specialised area of District Nursing. This position is available in the Chalfont\’s and Gerrards Cross area to join a newly formed team as a Community Nurse. We welcome highly motivated, effective team players committed to providing high-quality nursing services within the District Nursing field. We offer full skills development and a preceptorship programme. We are committed to promoting inclusion, where every staff member feels a sense of belonging. As a community nurse, you will be supported to become an autonomous practitioner, caring for patients with complex healthcare needs in their homes. Community Nursing Practice Community nursing is a specialised practice area. As a Community Staff Nurse, you will work within a locality team comprising District Nurse Team Leaders, Community Staff Nurses, Therapists, Assistant Practitioners, and Healthcare Assistants. Your role will involve caring for patients with complex needs in their homes and residential settings, including: Medication administration End of life care Managing deteriorating patients Wound care, including negative pressure and compression therapy Venepuncture Utilising advanced assessment and clinical skills You will work closely with social care, GPs, community therapies, and specialised services such as Tissue Viability, Diabetes, and Heart Failure.
