Commissioning Support Nurse NHS Midlands and Lancashire Commissioning Support Unit Join or sign in to find your next job Join to apply for the Commissioning Support Nurse NHS Midlands and Lancashire Commissioning Support Unit role at LNL-INFN About the role Due to extra funding, we have exciting opportunities for Registered Nurses to join a dynamic team providing commissioning services for individuals with complex care needs eligible for NHS Continuing Healthcare funding or NHS-funded nursing care. We encourage applications from serving personnel, reservists, veterans, and military families, recognizing their valuable contribution. Key responsibilities Implement policies related to NHS Funded Nursing Care and NHS Continuing Healthcare in Care Homes. Review eligibility for NHS Funded Nursing Care and Continuing NHS Care. Conduct nursing assessments for patients with health needs. Make recommendations based on assessments regarding patient needs. Refer patients for multidisciplinary team assessments when necessary. Identify at-risk clients and manage related issues. Report concerns about client care to relevant authorities. Maintain comprehensive assessment and care planning records. Identify and advise on specialist nursing equipment needs. Promote well-being and empower clients and carers, facilitating access to advocates when needed. Work with other professionals through joint assessments to meet health needs. Communicate assessment outcomes to relevant panels and teams. Provide advice on NHS funded care options to the public and professionals. Undertake retrospective assessments as needed. Collaborate with clients, families, and providers to arrange appropriate care funding. Maintain up-to-date knowledge of NHS guidelines and legal requirements. Participate in clinical governance, audits, research, and lifelong learning.
