Qualifications
- PT license in the State of Idaho
- Current CPR certification
- Valid Driver\’s License and Auto Insurance
- Home health and hospice experience preferred
Essential Job Functions/Responsibilities
- Provide physical therapy services according to a physician’s plan of care, including assessments, evaluations, and participation in care planning.
- Evaluate the home environment and assess muscle strength, mobility, gait, and range of motion to determine rehab potential.
- Direct physical therapy treatments and instruct patients and caregivers on therapeutic appliances.
- Prioritize patient needs and report treatment responses or condition changes to the physician.
- Initiate therapy programs and educate family or caregivers, participating in case conferences.
- Train patients in prosthetic device use when applicable.
- Identify additional home health service needs and refer patients as necessary.
- Prepare and submit clinical and progress summaries, participate in discharge planning.
- Provide consultation and in-service education to home health staff and peers.
- Supervise Physical Therapy Assistants per organizational and state regulations.
- Adhere to documentation, care standards, and professional conduct policies.
- Instruct, supervise, and evaluate home health aides when therapy is the sole skilled service, biweekly.
